The `tpg->tpg_nexus` pointer in the USB Target driver is dynamically
managed and tied to userspace configuration via ConfigFS. It can be
NULL if the USB host sends requests before the nexus is fully
established or immediately after it is dropped.
Currently, functions like `bot_submit_command()` and the data
transfer paths retrieve `tv_nexus = tpg->tpg_nexus` and immediately
dereference `tv_nexus->tvn_se_sess` without any validation. If a
malicious or misconfigured USB host sends a BOT (Bulk-Only Transport)
command during this race window, it triggers a NULL pointer
dereference, leading to a kernel panic (local DoS).
This exposes an inconsistent API usage within the module, as peer
functions like `usbg_submit_command()` and `bot_send_bad_response()`
correctly implement a NULL check for `tv_nexus` before proceeding.
Fix this by bringing consistency to the nexus handling. Add the
missing `if (!tv_nexus)` checks to the vulnerable BOT command and
request processing paths, aborting the command gracefully with an
error instead of crashing the system.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

In usbg_submit_command(), which handles the UAS protocol, the 'data_len'
member of the 'usbg_cmd' structure was left uninitialized by the
logic, relying instead on the zeroing performed by usbg_get_cmd().
In contrast, the BOT path (bot_submit_command) explicitly initializes
this field from the Command Block Wrapper (CBW). This discrepancy is
evident in error handling paths where __target_init_cmd() is called
using cmd->data_len.
Explicitly initialize cmd->data_len to 0 in the UAS path and use this
variable when calling target_submit_cmd(). This ensures architectural
consistency between the UAS and BOT protocols within the driver and
avoids reliance on implicit memory state during error transitions.

configfs_group_operations
'struct configfs_item_operations' and 'configfs_group_operations' are not
modified in these drivers.
Constifying these structures moves some data to a read-only section, so
increases overall security, especially when the structure holds some
function pointers.
On a x86_64, with allmodconfig, as an example:

The variable tpgt in usbg_make_tpg() is defined as unsigned long and is
assigned to tpgt->tport_tpgt, which is defined as u16. This may cause an
integer overflow when tpgt is greater than USHRT_MAX (65535). I
haven't tried to trigger it myself, but it is possible to trigger it
by calling usbg_make_tpg() with a large value for tpgt.
I modified the type of tpgt to match tpgt->tport_tpgt and adjusted the
relevant code accordingly.
This patch is similar to commit 59c816c1f24d ("vhost/scsi: potential
memory corruption").

Set TARGET_SCF_ACK_KREF flag and allow f_tcm to take the BOT command
reference. A usb request may be canceled, the f_tcm knows this. Let it
decides if the command should be freed. This is the same as how the UAS
interface is done.

Handle target_core_fabric_ops TASK MANAGEMENT functions and their
response. If a TASK MANAGEMENT command is received, the driver will
interpret the function TMF_*, translate to TMR_*, and fire off a command
work executing target_submit_tmr(). On completion, it will handle the
TASK MANAGEMENT response through uasp_send_tm_response().

If the transfer is cancelled due to a disconnect or driver tear down
(error code -ESHUTDOWN), then just free the command. However, if it got
cancelled due to other reasons, then send a sense CHECK CONDITION status
with TCM_CHECK_CONDITION_ABORT_CMD status to host notifying the delivery
failure. Note that this is separate from TASK MANAGEMENT function abort
task command, which will require a separate response IU.
See UAS-r04 section 8.

There may be different reasons for the transfer to be cancelled. Don't
blindly free the command without checking its status. We may still need
to properly respond to cancelled command. Check and only free the
command on endpoint disable.

Don't just wait for the data write completion and execute the target
command. We need to verify if the request completed successfully and not
just sending invalid data. The verification is done in the write request
completion routine. Queue the same work of the command to execute the
target_execute_cmd() on data write.

Resubmit command on completion to fetch more commands and service them
in parallel. Increase the number of work in a queue. Each work will be
for each command allowing them to be processed concurrently. Also, set
them to be unbounded by cpu to improve performance.

We can handle multiple commands concurently. Each command services a
stream id. At the moment, the driver will handle 32 outstanding streams,
which is equivalent to 32 commands. Make sure to allocate a matching
number of commands to the number of streams.

Host can assign stream ID value greater than number of streams
allocated. The tcm function needs to keep track of which stream is
available to assign the stream ID. This patch doesn't track that, but at
least it makes sure that there's no Oops if the host send tag with a
value greater than the number of supported streams.

Currently the default bMaxBurst is 0. Set default bMaxBurst to 15 (i.e.
16 bursts) to Data IN and OUT endpoints to improve performance. It
should be fine for a controller that supports less than 16 bursts. It
should be able to negotiate properly with the host at packet level for
the end of burst.
If the controller can't handle a burst of 16, and high performance isn't
important, the user can use BOT protocol from mass_storage gadget driver
instead.

We submitted the command with TARGET_SCF_ACK_KREF, which requires
acknowledgment of command completion. If the command fails, make sure to
decrement the ref count.

sysfs_emit()
Since snprintf() has the documented, but still rather strange trait of
returning the length of the data that *would have been* written to the
array if space were available, rather than the arguably more useful
length of data *actually* written, it is usually considered wise to use
something else instead in order to avoid confusion.
In the case of sysfs call-backs, new wrappers exist that do just that.
This patch replaces just one use of snprintf() found in the sysfs

In some cases, like with multiple LUN targets or where the target has to
respond to transport level requests from the receiving context it can be
better to defer cmd submission to a helper thread. If the backend driver
blocks on something like request/tag allocation it can block the entire
target submission path and other LUs and transport IO on that session.
In other cases like single LUN targets with storage that can support all
the commands that the target can queue, then it's best to submit the cmd
to the backend from the target's cmd receiving context.
Subsequent commits will allow the user to config what they prefer, but
drivers like loop can't directly submit because they can be called from a
context that can't sleep. And, drivers like vhost-scsi can support direct
submission, but need to keep their default behavior of deferring execution
to avoid possible regressions where the backend can block.
Make the drivers tell LIO core if they support direct submissions and their
current default, so we can prevent users from misconfiguring the system and
initialize devices correctly.

The following patches apply over Martin's 6.4 branches and Linus's tree.
They fix a couple regressions in iscsit that occur when there are TMRs
executing and a connection is closed. It also includes Dimitry's fixes in
related code paths for cmd cleanup when ERL2 is used and the write pending
hang during conn cleanup.
This version of the patchset brings it back to just regressions and fixes
for bugs we have a lot of users hitting. I'm going to fix isert and get it
hooked into iscsit properly in a second patchset, because this one was
getting so large. I've also moved my cleanup type of patches for a 3rd
patchset.

Allow target_get_sess_cmd() users to pass in the cmd counter they want to
use. Right now we pass in the session's cmd counter but in a subsequent
commit iSCSI will switch from per session to per conn.

target_submit_cmd() is now only for simple drivers that do their own sync
during shutdown and do not use target_stop_session(). It will never return
a failure, so we can remove that code from the driver.
Note: Before these patches target_submit_cmd() would never return an error
for usb since it does not use target_stop_session(). If it did then we
would have hit a refcount error here:
transport_send_check_condition_and_sense(se_cmd,
TCM_UNSUPPORTED_SCSI_OPCODE, 1);
transport_generic_free_cmd(&cmd->se_cmd, 0);
transport_send_check_condition_and_sense() calls queue_status and the
driver can sometimes do transport_generic_free_cmd() from there via
uasp_status_data_cmpl(). In that case, the above
transport_generic_free_cmd() would then hit a refcount error.
So that other use of the above error path in the driver is also probably
wrong, but someone with the hardware needs to fix that.

The first 5 descriptors are associated with BOT alternate setting,
and others are associated with UAS.
During handling UAS alternate setting f_tcm driver invokes
config_ep_by_speed and this function sets incorrect companion endpoint
descriptor in usb_ep object.
Instead setting ep->comp_desc to uasp_bi_ep_comp_desc function in this
case set ep->comp_desc to bot_uasp_ss_bi_desc.
And in result it uses the descriptor from BOT alternate setting
instead UAS.
Finally, it causes that controller driver during enabling endpoints
detect that just enabled endpoint for bot.

If a memory allocation fails within a 'usb_ep_alloc_request()' call, the
already allocated memory must be released.

Initialization of orig_fe_lun is moved to transport_init_se_cmd() from
transport_lookup_cmd_lun(). This helps for the cases where the SCSI request
fails before the call to transport_lookup_cmd_lun() so that
trace_target_cmd_complete() can print the LUN information to the trace
buffer. Due to this change, the lun parameter is removed from
transport_lookup_cmd_lun() and transport_lookup_tmr_lun().

Set the request->is_last to each stream request to indicate that the
request is the last stream request of a transfer. The DWC3 controller
needs to know this info to properly switch streams. The current
implementation of f_tcm uses a single request per transfer, so every
stream request is the last of its stream.
