scsi: remove eh_timed_out methods in the transport template

Instead define the timeout behavior purely based on the host_template eh_timed_out method and wire up the existing transport implementations in the host templates. This also clears up the confusion that the transport template method overrides the host template one, so some drivers have to re-override the transport template one. BUG: headers with BUG/BUG_ON etc. need linux/bug.h

If a header file is making use of BUG, BUG_ON, BUILD_BUG_ON, or any other BUG variant in a static inline (i.e. not in a #define) then that header really should be including and not just expecting it to be implicitly present. We can make this change risk-free, since if the files using these headers didn't have exposure to linux/bug.h already, they would have been causing compile failures/warnings. block: unify request timeout handling

Right now SCSI and others do their own command timeout handling. Move those bits to the block layer. Instead of having a timer per command, we try to be a bit more clever and simply have one per-queue.
