refcount_t type and corresponding API should be used instead of atomic_t when the variable is used as a reference counter. This allows to avoid accidental refcounter overflows that might lead to use-after-free situations. cipso: don't use IPCB() to locate the CIPSO IP option

Using the IPCB() macro to get the IPv4 options is convenient, but unfortunately NetLabel often needs to examine the CIPSO option outside of the scope of the IP layer in the stack. While historically IPCB() worked above the IP layer, due to the inclusion of the inet_skb_param struct at the head of the {tcp,udp}_skb_cb structs, recent commit 971f10ec ("tcp: better TCP_SKB_CB layout to reduce cache line misses") reordered the tcp_skb_cb struct and invalidated this IPCB() trick. This patch fixes the problem by creating a new function, cipso_v4_optptr(), which locates the CIPSO option inside the IP header without calling IPCB(). Unfortunately, this isn't as fast as a simple lookup so some additional tweaks were made to limit the use of this new function. When CONFIG_NETLABEL is disabled, the cipso_v4_validate() function could loop forever in the main loop if opt[opt_iter +1] == 0, this will causing a kernel crash in an SMP system, since the CPU executing this function will stall /not respond to IPIs. cipso: handle CIPSO options correctly when NetLabel is disabled

When NetLabel is not enabled, e.g. CONFIG_NETLABEL=n, and the system receives a CIPSO tagged packet it is dropped (cipso_v4_validate() returns non-zero). In most cases this is the correct and desired behavior, however, in the case where we are simply forwarding the traffic, e.g. acting as a network bridge, this becomes a problem. This patch fixes the forwarding problem by providing the basic CIPSO validation code directly in ip_options_compile() without the need for the NetLabel or CIPSO code. The new validation code can not perform any of the CIPSO option label/value verification that cipso_v4_validate() does, but it can verify the basic CIPSO option format.
