Prepare for the coming implementation by GCC and Clang of the __counted_by
attribute. Flexible array members annotated with __counted_by can have
their accesses bounds-checked at run-time checking via CONFIG_UBSAN_BOUNDS
(for array indexing) and CONFIG_FORTIFY_SOURCE (for strcpy/memcpy-family
functions).

As found with Coccinelle[1], add __counted_by for
struct v4l2_subscribed_event.
Additionally, since the element count member must be set before accessing
the annotated flexible array member, move its initialization earlier.

When unregistering a V4L2 device node, make sure any filehandles
that are waiting for an event are woken up.

Add v4l2_event_wake_all() to v4l2-event.c and call it from
video_unregister_device().

Otherwise userspace might never know that a device node was removed.

Internally use ktime_get_ns() to get the timestamp of the event.
Only convert to timespec when interfacing with userspace.

Both v4l2-event.rst and v4l2-event.h have an overview of
events, but there are some inconsistencies there:

- at v4l2-event, the event's ring buffer is called kevent. Its
  name is, instead, v4l2_kevent;

- Some things are mentioned on both places (with different words),
  others are either on one of the files.

In order to cleanup this mess, put everything at v4l2-event.rst
and improve it to be a little more coherent and to have cross
references.

Drop the FSF's postal address from the source code files that typically
contain mostly the license text. Of the 628 removed instances, 578 are
outdated.
