All these files have been updated in the commit given in the Fixes: tag
below.

When the SPDX-License-Identifier: has been added, the corresponding text at
the beginning of the files has not been deleted.
All these texts are about GPL-2.0+, with different variation in the
wording.

Remove these now useless lines to save some LoC.

The HNP work can be re-scheduled while it's still in-fly. This results in
re-initialization of the busy work, resetting the hrtimer's list node of
the work and crashing kernel with null dereference within kernel/timer
once work's timer is expired. It's very easy to trigger this problem by
re-plugging USB cable quickly. Initialize HNP work only once to fix this
trouble.

<title>USB: add SPDX identifiers to all remaining files in drivers/usb/</title>

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

If usb/otg-fsm.h and usb/composite.h are included together
then it results in the build warning [1].

Prevent that by defining VDBG locally.

Also get rid of MPC_LOC which doesn't seem to be used
by anyone.

struct otg_fsm is the interface to the OTG state machine.

Document the input, output and internal state variables.
Definations are taken from Table 7-2 and Table 7-4 of
the USB OTG &amp; EH Specification Rev.2.0

Re-arrange some of the members as per use case for more
clarity.

Add A-idle to B-disconnect timer, B-device detects that bus is idle
for more than TB_AIDL_BDIS min and begins HNP by turning off pullup
on D+. This allows the bus to discharge to the SE0 state.

Adds HNP polling timer when transits to host state, the OTG status
request will be sent to peripheral after timeout, if host request flag
is set, it will switch to peripheral state, otherwise it will repeat HNP
polling every 1.5s and maintain the current session.
