With the inclusion of atomic FETCH-OP variants, many places in the
kernel can make use of atomic_fetch_$op() to avoid the callers that
need to compute the value/state _before_ the operation.

Peter Zijlstra laid out the machinery but we are still missing the
simpler dec,inc() calls (which future patches will make use of).

This patch only deals with the generic code, as at least right now
no arch actually implement them -- which is similar to what the
OP-RETURN primitives currently do.

We should only expand the atomic64 relaxed bits once we've included
all relevant headers. So move it down until after we potentially
include asm-generic/atomic64.h.

In practise this will not have made a difference so far, since the
generic bits will not define _relaxed versions.

This patch functionally reverts:

  5fd7a09cfb8c ("atomic: Export fetch_or()")

During the merge Linus observed that the generic version of fetch_or()
was messy:

" This makes the ugly "fetch_or()" macro that the scheduler used
internally a new generic helper, and does a bad job at it. "

  e23604edac2a Merge branch 'timers-nohz-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip

Now that we have introduced atomic_fetch_or(), fetch_or() is only used
by the scheduler in order to deal with thread_info flags which type
can vary across architectures.

Lets confine fetch_or() back to the scheduler so that we encourage
future users to use the more robust and well typed atomic_t version
instead.

While at it, fetch_or() gets robustified, pasting improvements from a
previous patch by Ingo Molnar that avoids needless expression
re-evaluations in the loop.

This is deemed to replace the type generic fetch_or() which brings a lot
of issues such as macro induced block variable aliasing and sloppy types.
Not to mention fetch_or() doesn't refer to any namespace, adding even
more confusion.

So lets provide an atomic_t version. Current and next users of fetch_or()
are thus encouraged to use atomic_t.

Some architectures may have their special barriers for acquire, release
and fence semantics, so that general memory barriers(smp_mb__*_atomic())
in the default __atomic_op_*() may be too strong, so allow architectures
to define their own helpers which can overwrite the default helpers.

By checking huge_pte_none(), we incorrectly classify PTE markers as
"present". Instead, check huge_pte_none_mostly(), classifying PTE markers
the same as if the PTE were completely blank.

PTE markers, unlike other kinds of swap entries, don't reference any
physical page and don't indicate that a physical page was mapped
previously. As such, treat them as non-present for the sake of mincore().

Convert to struct mnt_idmap.

Last cycle we merged the necessary infrastructure in
256c8aed2b42 ("fs: introduce dedicated idmap type for mounts").
This is just the conversion to struct mnt_idmap.

Currently we still pass around the plain namespace that was attached to a
mount. This is in general pretty convenient but it makes it easy to
conflate namespaces that are relevant on the filesystem with namespaces
that are relevent on the mount level. Especially for non-vfs developers
without detailed knowledge in this area this can be a potential source for
bugs.

Once the conversion to struct mnt_idmap is done all helpers down to the
really low-level helpers will take a struct mnt_idmap argument instead of
two namespace arguments. This way it becomes impossible to conflate the two
eliminating the possibility of any bugs. All of the vfs and all filesystems
only operate on struct mnt_idmap.

This patch still does not use pte marker in any way, however it teaches
the core mm about the pte marker idea.

For example, handle_pte_marker() is introduced that will parse and handle
all the pte marker faults.

Many of the places are more about commenting it up - so that we know
there's the possibility of pte marker showing up, and why we don't need
special code for the cases.

Patch series "MM changes to improve swap-over-NFS support".

Assorted improvements for swap-via-filesystem.

This is a resend of these patches, rebased on current HEAD. The only
substantial changes is that swap_dirty_folio has replaced
swap_set_page_dirty.

Currently swap-via-fs (SWP_FS_OPS) doesn't work for any filesystem. It
has previously worked for NFS but that broke a few releases back. This
series changes to use a new ->swap_rw rather than ->readpage and
->direct_IO. It also makes other improvements.

There is a companion series already in linux-next which fixes various
issues with NFS. Once both series land, a final patch is needed which
changes NFS over to use ->swap_rw.

This patch (of 10):

Many functions declared in include/linux/swap.h are only used within mm/

Create a new "mm/swap.h" and move some of these declarations there.
Remove the redundant 'extern' from the function declarations.
