USB: fhci-hcd: no need to check return value of debugfs_create functions

When calling debugfs functions, there is no need to ever check the return value. The function can work or not, but the code logic should never do something different based on this. There is also no need to keep the file dentries around at all, so remove those variables from the host controller structure. Now that the SPDX tag is in all USB files, that identifies the license in a specific and legally-defined manner. So the extra GPL text wording can be removed as it is no longer needed at all. This is done on a quest to remove the 700+ different ways that files in the kernel describe the GPL license text. And there's unneeded stuff like the address (sometimes incorrect) for the FSF which is never needed. No copyright headers or other non-license-description text was removed. It's good to have SPDX identifiers in all files to make it easier to audit the kernel tree for correct licenses. Update the drivers/usb/ and include/linux/usb* files with the correct SPDX license identifier based on the license text in the file itself. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text. USB: FHCI: upgrade the isochronous API

This patch attempts to fix the isochronous API in the fhci-hcd driver. There are two problems with the current code: ed->last_iso is used but not set anywhere. The patch changes its name to ed->next_iso and uses it to store the frame number of the next available slot in the isochronous stream. urb->start_frame isn't set when the URB_ISO_ASAP flag is off. The patch sets it to the next available slot if the stream is in use, or the current frame otherwise. This won't give the right behavior when an underrun occurs, but I don't know enough about the driver to handle that case. Unfortunately, I don't have any way to test these changes. USB: FHCI: Reusing QUICC Engine USB Controller registers from immap_qe.h

The struct fhci_regs (in drivers/usb/host/fhci.h) is basically a redefinition of the struct qe_usb_ctlr (in arch/powerpc/include/asm/immap_qe.h). The qe_usb_ctlr struct is preferrable once it uses accurately the registers' names found in the Freescale's QUICC Engine Block Reference Manuals (QEIWRM.pdf Rev.4.4 Chapter 19 for MPC836xE series and MPC8323ERM.pdf Rev.2 Chapter 36 for MPC832xE series), making easier to map the FHCI device driver to the hardware manual. Also, as the FHCI driver uses the USB Controller registers, the name qe_usb_ctlr is a more precise representation of the hardware than fhci_regs.
