When the tty layer requests the uart to throttle, the current code
executing in msm_serial will trigger "Bad mode in Error Handler" and
generate an invalid stack frame in pstore before rebooting (that is if
pstore is indeed configured: otherwise the user shall just notice a
reboot with no further information dumped to the console).

This patch replaces the PIO byte accessor with the word accessor
already used in PIO mode.

Due to the complexities of modern Qualcomm SoCs, about a half dozen drivers
must successfully probe before the clocks for the console are present, and
the console can successfully probe.  Depending on several random factors
such as probe order and modules vs builtin, msm_serial may not be able to
successfully probe for some, at which point, __init annotated functions
may become unmapped.  If this occurs, msm_console_setup() will be called
from the probe path, but will no longer exist, resulting in a kernel
panic.

Resolve this issue by removing the __init annotation from
msm_console_setup().

Now that the SPDX tag is in all tty files, that identifies the license
in a specific and legally-defined manner.  So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/tty files files with the correct SPDX license
identifier based on the license text in the file itself.  The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

Move the request_irq() call to the end of the msm_startup(),
so that we don't handle interrupts while msm_startup() is
running. This avoids potential races while initialization
is in progress. For example, consider below scenario
where rx handler reads the intermediate value of dma-&gt;chan,
set in msm_request_rx_dma(), and tries to do dma mapping,
which results in data abort.

uart_port_startup()
  msm_startup()
   request_irq()
   ...
   msm_request_rx_dma()
    ...
    dma-&gt;chan = dma_request_slave_channel_reason(dev, "rx");
    &lt;UART RX IRQ&gt;

If the driver is built as a module, autoload won't work because the module
alias information is not filled. So user-space can't match the registered
device with the corresponding module.

Export the module alias information using the MODULE_DEVICE_TABLE() macro.

Before this patch:

$ modinfo drivers/tty/serial/msm_serial.ko | grep alias
$

After this patch:

$ modinfo drivers/tty/serial/msm_serial.ko | grep alias

For dm uarts in pio mode tx data is transferred to the fifo register 4
bytes at a time, but care is not taken when these 4 bytes spans the end
of the xmit buffer so the loop might read up to 3 bytes past the buffer
and then skip the actual data at the beginning of the buffer.

Fix this by, analogous to the DMA case, make sure the chunk doesn't
wrap the xmit buffer.
