sparse complains on this code about casts that lose bits
due to the usage of bitwise not, but really we do want
16 bits only, so clarify that by using masks.

There are still surprisingly many non-chanctx drivers, but in
mac80211 that code is a bit awkward. Simplify this by having
those drivers assign 'emulated' ops, so that the mac80211 code
can be more unified between non-chanctx/chanctx drivers. This
cuts the number of places caring about it by about 15, which
are scattered across - now they're fewer and no longer in the
channel context handling.

bcm4331 seems to not function correctly with QoS support. This may be due
to issues with currently available firmware or potentially a device
specific issue.

When queues that are not of the default "best effort" priority are
selected, traffic appears to not transmit out of the hardware while no
errors are returned. This behavior is present among all the other priority
queues: video, voice, and background. While this can be worked around by
setting a kernel parameter, the default behavior is problematic for most
users and may be difficult to debug. This patch offers a working out-of-box
experience for bcm4331 users.

ieee80211 queue since there is only one queue. Stop queue 0 when QoS is
disabled to prevent trying to stop a non-existent queue and failing to stop
the actual queue instantiated.

make htmldocs warns:

Documentation/driver-api/80211/mac80211:109: ./include/net/mac80211.h:5170: WARNING: Duplicate C declaration, also defined at mac80211:1117.
Declaration is '.. c:function:: void ieee80211_tx_status (struct ieee80211_hw *hw, struct sk_buff *skb)'.

This is because there's a function named ieee80211_tx_status() and a struct named
ieee80211_tx_status. This has been discussed previously but no solution found:

There's also a bug open for three years with no solution in sight:

So I guess we have no other solution than to a workaround this in the code,
for example to rename the function to ieee80211_tx_status_skb() to avoid the
name conflict. I got the idea for the name from ieee80211_tx_status_noskb() in
which the skb is not provided as an argument, instead with
ieee80211_tx_status_skb() the skb is provided.

Compile tested only.

clang warns about an unpacked structure inside of a packed one:

drivers/net/wireless/broadcom/b43/b43.h:654:4: error: field data within 'struct b43_iv' is less aligned than 'union (unnamed union at /home/arnd/arm-soc/drivers/net/wireless/broadcom/b43/b43.h:651:2)' and is usually due to 'struct b43_iv' being packed, which can lead to unaligned accesses [-Werror,-Wunaligned-access]

The problem here is that the anonymous union has the default alignment
from its members, apparently because the original author mixed up the
placement of the __packed attribute by placing it next to the struct
member rather than the union definition. As the struct itself is
also marked as __packed, there is no need to mark its members, so just
move the annotation to the inner type instead.

As Michael noted, the same problem is present in b43legacy, so
change both at the same time.

Commit 399500da18f7 ("ssb: pick PCMCIA host code support from b43 driver")
removes the config B43_PCMCIA.

Clean up the last reference to this removed config B43_PCMCIA in the
b43_print_driverinfo() function.

mac80211 is fully switching over to the internal TX queue (iTXQ)
implementation. Update all drivers not yet providing the now mandatory
wake_tx_queue() callback.

As an side effect the netdev interfaces of all updated drivers will
switch to the noqueue qdisc.
