Use the normal kernel debugging mechanism which also
enables dynamic_debug at the same time.

Other miscellanea:

o Remove sysctl for irda_debug
o Remove function tracing like uses (use ftrace instead)
o Coalesce formats
o Realign arguments
o Remove unnecessary OOM messages

None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>. Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

This covers everything under drivers/net except for wireless, which
has been submitted separately.

Straight forward conversions to CONFIG_MODULE; many drivers
include <linux/kmod.h> conditionally and then don't have any
other conditional code so remove it from those.

Remove includes of <linux/smp_lock.h> where it is not used/needed.
Suggested by Al Viro.

Builds cleanly on x86_64, i386, alpha, ia64, powerpc, sparc,
sparc64, and arm (all 59 defconfigs).

Semaphore to mutex conversion.

The conversion was generated via scripts, and the result was validated
automatically via a script as well.

EXPORT_SYMBOL's do nowadays belong to the files where the actual
functions are.

Moving the module_init/module_exit to the file with the actual functions
has the advantage of saving a few bytes due to the removal of two
functions.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
