drivers/net: remove all references to obsolete Ethernet-HOWTO

This howto made sense in the 1990s when users had to manually configure ISA cards with jumpers or vendor utilities, but with the implementation of PCI it became increasingly less and less relevant, to the point where it has been well over a decade since I last updated it. And there is no value in anyone else taking over updating it either. However the references to it continue to spread as boiler plate text from one Kconfig file into the next. We are not doing end users any favours by pointing them at this old document, so lets kill it with fire, once and for all, to hopefully stop any further spread. We should prefer `struct pci_device_id` over `DEFINE_PCI_DEVICE_TABLE` to meet kernel coding style guidelines. This issue was reported by checkpatch. None of these files are actually using any __init type directives and hence don't need to include . Most are just a left over from __devinit and __cpuinit removal, or simply due to code getting copied from one driver to the next. __GFP_ZERO is an uncommon flag and perhaps is better not used. static inline dma_zalloc_coherent exists so convert the uses of dma_alloc_coherent with __GFP_ZERO to the more common kernel style with zalloc. Remove memset from the static inline dma_zalloc_coherent and add just one use of __GFP_ZERO instead. Trivially reduces the size of the existing uses of dma_zalloc_coherent. net: Move MII out from under NET_CORE and hide it

All drivers that select MII also need to select NET_CORE because MII depends on it. This is a bit ridiculous because NET_CORE is just a menu option that doesn't enable any code by itself. The driver core clears the driver data to NULL after device_release or on probe failure, since commit 0998d0631001288a5974afc0b2a5f568bcdecb4d (device-core: Ensure drvdata = NULL when no driver is bound). Thus, it is not needed to manually clear the device driver data to NULL.
