drivers/net: remove all references to obsolete Ethernet-HOWTO

This howto made sense in the 1990s when users had to manually configure ISA cards with jumpers or vendor utilities, but with the implementation of PCI it became increasingly less and less relevant, to the point where it has been well over a decade since I last updated it. And there is no value in anyone else taking over updating it either. However the references to it continue to spread as boiler plate text from one Kconfig file into the next. We are not doing end users any favours by pointing them at this old document, so lets kill it with fire, once and for all, to hopefully stop any further spread. We should prefer `struct pci_device_id` over `DEFINE_PCI_DEVICE_TABLE` to meet kernel coding style guidelines. This issue was reported by checkpatch. All drivers that select MII also need to select NET_CORE because MII depends on it. This is a bit ridiculous because NET_CORE is just a menu option that doesn't enable any code by itself. net: vlan: add protocol argument to packet tagging functions

Add a protocol argument to the VLAN packet tagging functions. In case of HW tagging, we need that protocol available in the ndo_start_xmit functions, so it is stored in a new field in the skb. Rename the hardware VLAN acceleration features to include "CTAG" to indicate that they only support CTAGs. drivers/net: fix up function prototypes after __dev* removals

The __dev* removal patches for the network drivers ended up messing up the function prototypes for a bunch of drivers. This patch fixes all of them back up to be properly aligned.
