scsi: flip the default on use_clustering

Most SCSI drivers want to enable "clustering", that is merging of segments so that they might span more than a single page. mpt_attach() and mptfc_probe() are never called in atomic context. They call kzalloc() and kcalloc() with GFP_ATOMIC, which is not necessary. GFP_ATOMIC can be replaced with GFP_KERNEL. scsi: remove eh_timed_out methods in the transport template

Instead define the timeout behavior purely based on the host_template eh_timed_out method and wire up the existing transport implementations in the host templates. This also clears up the confusion that the transport template method overrides the host template one, so some drivers have to re-override the transport template one. The workqueue "fc_rescan_work_q" queues multiple work items viz &ioc->fc_rescan_work, &ioc->fc_lsc_work, &ioc->fc_setup_reset_work, which require strict execution ordering. Hence, an ordered dedicated workqueue has been used. WQ_MEM_RECLAIM has been set since the workqueue is belongs to a storage driver which is being used on a memory reclaim path and hence, requires forward progress under memory pressure.
