lightnvm: disable the subsystem

In commit 9ea9b9c48387 ("remove the lightnvm subsystem") the lightnvm subsystem was removed as there is no hardware in the wild for it, and the code is known to have problems. This should also be disabled for older LTS kernels as well to prevent anyone from accidentally using it. lightnvm: fix memory leak when submit fails

The allocated page is not released if error occurs in nvm_submit_io_sync_raw(). __free_page() is moved ealier to avoid possible memory leak issue. lightnvm: fix out-of-bounds write to array devices->info[]

There is an off-by-one array check that can lead to a out-of-bounds write to devices->info[i]. Fix this by checking by using >= rather than > for the size check. Also replace hard-coded array size limit with ARRAY_SIZE on the array. block: move ->make_request_fn to struct block_device_operations

The make_request_fn is a little weird in that it sits directly in struct request_queue instead of an operation vector. Replace it with a block_device_operations method called submit_bio (which describes much better what it does).
