The legay ide driver has been replace with libata starting in 2003 and has
been scheduled for removal for a while.  Finally kill it off so that we
can start cleaning up various bits of cruft it forced on the block layer.

The "drive-&gt;dn" value is a u8 and it is controlled by root only, but
it could be out of bounds here so let's check.

Add SPDX license identifiers to all files which:

 - Have no license information of any form

 - Have MODULE_LICENCE("GPL*") inside which was used in the initial
   scan/conversion to ignore the file

These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:

  GPL-2.0-only

CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Just use the standard ide_timing_compute() helper to calculate
PIO and MWDMA timings.  This fixes some issues with the open-coded
version like allowing faster MWDMA timings than the ones required
by the current PIO mode or not accounting for the enhanced MWDMA
cycle time specified by the device.

Based on libata pata_cmd64x host driver.
