Some files functions in 'arch/powerpc/sysdev' are deserving of an `__init`
macro attribute. These functions are only called by other initialization
functions and therefore should inherit the attribute.
Also, change function declarations in header files to include `__init`.

ipic_set_priority() has been unused since 2006 when the last usage was
removed in commit b9f0f1bb2bca ("[POWERPC] Adapt ipic driver to new
host_ops interface, add set_irq_type to set IRQ sense").

Now that arch/ppc is gone and CONFIG_PPC_MERGE is always set, remove
the dead code associated with !CONFIG_PPC_MERGE from arch/powerpc
and include/asm-powerpc.

mkdir arch/powerpc/include/asm
git mv include/asm-powerpc/* arch/powerpc/include/asm

Followed by a few documentation/comment fixups and a couple of places
where &lt;asm-powepc/...&gt; was being used explicitly.  Of the latter only
one was outside the arch code and it is a driver only built for powerpc.
