nios2: Replace ASSEMBLY with ASSEMBLER in non-uapi headers

While the GCC and Clang compilers already define __ASSEMBLER__ automatically when compiling assembly code, __ASSEMBLY__ is a macro that only gets defined by the Makefiles in the kernel. This can be very confusing when switching between userspace and kernelspace coding, or when dealing with uapi headers that rather should use __ASSEMBLER__ instead. So let's standardize on the __ASSEMBLER__ macro that is provided by the compilers now. This is a completely mechanical patch (done with a simple "sed -i" statement). vdso: Introduce vdso/page.h

Most architectures only support a single hardcoded page size. In order to ensure that each one of these sets the corresponding Kconfig symbols, change over the PAGE_SHIFT definition to the common one and allow only the hardware page size to be selected. Every architecture that supports FLATMEM memory model defines its own version of pfn_valid() that essentially compares a pfn to max_mapnr. Use mips/powerpc version implemented as static inline as a generic implementation of pfn_valid() and drop its per-architecture definitions. mm/vma: define a default value for VM_DATA_DEFAULT_FLAGS

There are many platforms with exact same value for VM_DATA_DEFAULT_FLAGS This creates a default value for VM_DATA_DEFAULT_FLAGS in line with the existing VM_STACK_DEFAULT_FLAGS. While here, also define some more macros with standard VMA access flag combinations that are used frequently across many platforms. Apart from simplification, this reduces code duplication as well. When compiling with 'W=1' warnings such as the following occur all over the place: ./include/linux/pfn.h:21:22: warning: comparison of unsigned expression >= 0 is always true [-Wtype-limits] which is due to ARCH_PFN_OFFSET being 0 by default (if CONFIG_NIOS2_MEM_BASE is not changed in the Kconfig). To align with other architectures, the expression produced by expanding the macro page_to_virt() should be of type void*, since it returns a virtual address.
